Requesting Contact Form Changes

Whenever possible, we encourage iCarol Admins to make their own changes to their contact forms.  iCarol Admins have access to the tools and training materials to complete contact form edits themselves, and we want to empower our clients to do so.  However, if you would like the assistance of the iCarol Support team to make contact forms edits, please submit a ticket articulating your needs as clearly and specifically as possible.  Here are some guidelines to use.
  1. If multiple edits are being requested, list them in the order they appear on the contact form.
  2. Clearly indicate the name of the contact form to be edited
  3. Clearly indicate the Category Name, Group Name and Field Name involved in the change request so the iCarol Support Team member can find the correct area to edit.  Category names appear in bold on the contact form, and are used to group together similar questions.  Groups are the questions that appear in each Category.  They can appear as a drop-down list, a check-box list or an open text box.  Fields are the options that appear in drop-down and check-box Groups.
  4. Clearly indicate what action you are requesting, for example, add, delete, deactivate, move, or re-name.
  5. For add and move requests, clearly indicate what is being added or moved (indicate Category, Group or Field name), and where it is being added or moved to (ie. above/below already existing Categories, Groups or Fields)
  6. When adding a new Group, indicate the format of the Group (drop-down, check-box or open text) and if a drop-down or check-box Group, include all Fields to be added, in the order they should be added
  7. Indicate any specific settings you want applied to any Category, Group or Field.  Some examples are hide in a 'click here' box, 2 columns for Categories, make required, initial default selection, if selected all required fields are not required, etc.
  8. If adding conditional Groups or Categories, indicate which Field triggers the conditional

ADD Group: Calling on Behalf (drop-down format, required)
Fields: Self (default selection), Other
Location: Under 'Client Status'

Contact Form Name: 211 Call Report

ADD Conditional Group: Calling on Behalf, Other (open text format, required)
Location: under Group: Calling on Behalf
Trigger: when 'Other' is selected in Category: Demographics, Group: Calling on Behalf

Note that requesting contact form edits from the iCarol Support Team may carry a fee.  The iCarol Support Team will let you know if this is the case before any edits are completed and provide a quote.  The edits will be completed after the quote is accepted.
